Most likely it is spark plug wires. If you put it in gear and load it up (Hold the brake and press the throttle at the same time) and it starts skipping/misfiring, chances are that your spark plug wires are breaking down. To confirm, at night, raise the hood and do this. Have someone look at the engine when you "load it up". If they see any sparks around the engine, it's the plug wires. Do yourself a favor and buy good wires. It is hard enough to replace them on a van and a job you don't want to do again in 2 weeks. (While you are in there, it is a good time to replace the plugs too.)
Need to check the EGR valve. It sounds like it is opening to quick.
