  1. Compile a letter in which you explicitly state you are requesting a birth report, of the FS-240. In here, you should include the reasons for your request, the date of birth, how many copies you require and both your current phone number and address.
  2. The letter should also entail specifics regarding the birth child, such as all known names he or she may have acquired through birth, death, marriage or adoption, as well as any relevant dates, birth, death and marriage, any locations and both parents’ names, dates and places of births.
