Step 1: Do not scrimp on fabric. Find yourself a good-quality fabric in a soft cotton or cotton blend. Flannel makes an excellent choice for the bottom of the duvet, as it is available in a multitude of colors and prints.
Step 2: Measure your duvet or comforter and add 3 ¾ inches to the length and 1 ½ inches to the width to allow for the seams and hems.
Step 3: When choosing colors and fabrics, it would be a good idea to have contrasting patterns and colors, so that it can be reversible. Be brave and live it up a little.
Step 4: Once you have selected your fabric, place the right side of the fabrics back to back. Sew the pieces of fabric together with a ¼ inch seam along the sides and leave approximately a half inch along the top for a seam or closure.
Step 5: If you want a non-traditional look, use buttons, hooks or ribbons for the top closure of the cover instead of zippers or snaps.

Covers are much easier to clean than the duvet or comforter itself, as it is more easily handled and fits better into your home washing machine. Measure the duvet and add about four inches to top, bottom and sides so suitable seams and closure can be created. The finished cover should be the same size as the duvet or slightly larger for best appearance.
A cover can be made from fabric purchased from a store or from two flat sheets. White sales will yield good prices on sheets, but buy those with a good thread count. The avid bargain hunter can find quality and very inexpensive sheets at a resale store like Goodwill. Choose sheets or fabric in a color and style that will work with the room.
It is also possible to create a unique design by purchasing plain fabric or sheets and using permanent markers that are alcohol-based to draw a design that will coordinate with the room. Experiment first to find a design that will be pleasing. Outline it with markers. Leaving about one fourth of an inch between lines, fill in spaces as desired.
On a dry day, take the fabric outdoors. Wear old clothes because this process can stain them. Put 70 percent rubbing alcohol in a spray bottle, and wet the fabric. The colors will run together and blend.
Allow the fabric to air dry, which will happen pretty quickly. Bring it indoors and iron it at the setting for cotton blends. After this, it should be laundered with gentle detergent such as Delicare. Dry in the dryer and press if needed.
Put the two pieces together with right sides in, then stitch together with a half-in seam, leaving one end open. Add a second seam for strength. Trim the seam to a fourth of an inch. Clip corners and turn. Or make French seams. Sew in a zipper of the proper length on the open end.
The new duvet cover will be easy to remove for occasional laundering. If desired, use a button closure with buttonholes instead. This takes more time and will be less convenient, but the duvet cover does not need laundering too often if sheets are used with it.

The answer is - there IS no standard! Duvets can come in quite a range of sizes so the duvet cover needs to fit the size of your particular duvet. The first step would be to measure your duvet itself. Some queen sized duvets are actually called full/queen or double/queen which means they are meant to fit both a double bed and a queen sized bed. In my experience, any duvet so labeled is usually a bit smaller than one labeled just queen. I clicked around to find some examples of the measurements of duvets and duvet covers. I looked at sites with directions for how to sew your own cover and at sites which showed ready made duvet covers from well-known stores and here are some examples I came across. You will see that there is a very large variation in sizes quoted so for certain it's buyer beware.
