It depends what country you're in. Here in the UK, we have a 'national health service' which is free for everyone who has no means of paying for treatment. If someone has an infected tooth here, they would either see their own dentist - or present themselves at the nearest hospital's Accident and Emergency department. Every A & E dept. has a resident emergency dentist.
