First you have to have beaten the Deku Tree. Then, you need to go to the Lost Woods. If you don't know where it is, there should be a ledge to the left (or right if differently positioned) of the exit to Hyrule Field: opposite exit to the Deku Tree. Anyway, once in the Lost Woods, follow the song to the Sacred Forest Meadow. In other words, walk up to each path and determine which path when near plays the song (Saria's Song) the loudest. The rest is simple. Fight the Wolfos, and go through the maze. It's a simple maze. There's only one split, so you should be able to get through it just fine on your own.
You complete the forest temple and Saria will get reawaken and she will give you the Forest Medalion.
