This very common symptom is typical of a failed blower motor resistor. Easy fix and about $40. It is up in the blower duct behind the glove box and is accessed from below. Remove the lower kick panel and drop the glove box forward and out of the way so you can see the electrical connection to the blower motor.

Its not the relay, it is the blower motor resistor.
The common cause is a failed blower resistor.
It sounds like the blower resistor assembly is defective. All fan speeds except for high go through resistors (that is what controls the speeds from low to high). Number 4 is high and has the full 12 volts going to the fan.
I've had a similar problem on my 98 civic ex. There's two resistors wired to the fan control. One for speeds 1 and 2 and another for speeds 3 and 4. I suspect the one controling the 1 and 2 speeds has blown.
Possibly a fan resistor. Does it work on only certain speeds like 1 and 2 but not 3 and 4?
Check the blower motor resistor as it controls the fan speeds. Also check the blower motor as it will burn out resistors if its drawing to much amperage
Most heater blaower motors have 2 relays: 1 for speeds 1-4 and a high speed fan relay for 5th speed I do not know what kind of vehicle or location of relays Most heater blaower motors have 2 relays: 1 for speeds 1-4 and a high speed fan relay for 5th speed I do not know what kind of vehicle or location of relays
Your fan motor is going out.

Blower Motor Resistor Pack is defective.
A 4-speed fan switch typically has settings for low, medium-low, medium-high, and high speeds. These settings can be adjusted by turning the switch to the desired speed level. The fan speed will change accordingly based on the selected setting.
