Had the same trouble with a Toyota Tarago, door was locked and unable to unlock. So had to cut hole on external of door to unlock.
try turning the key 90 degrees anti-clockwise then hold it in that position whilst you pull the catch on the handle to open it. Mine had this anti-theft modification on the side door as it is and ex-BT van and it took me aged to find out.

if rear side door; unlock and pull handle. if boot lid; pull handle in drivers side door-well (side of the drivers seat) "door open"
