look at how much the engine moves, if moves more than 1" fore-aft, replace. if you can see the rubber in the mount and it is torn, replace. replace the lower mount too
The year, make, model and engine wasn't mentioned but in general you remove the motor mount anchor bolt(s), lift the engine a few inches and support it safely, unbolt motor mount(s) from engine, remove mount(s), replace mount(s) and finish in reverse order.

You need to jack it up enough to take the weight of of the mount.
Purchase your replacement part. Put a jack under the engine with a board to support the engine. Put slight pressure on the engine but no more. Remove the mount and replace. Lower jack.
First loosen the 2 bolts that hold the mount on. Then remove the mount. Attach one end of a ratchet to the supercharger nose(it will hold and not hurt the nose) or any other bracket in the back, then attach the other end of ratchet to the front end of car. Attach the new engine mount onto the engine and then tighten the ratchet strap to bring the engine forward until the new motor mount lines up with the bolt holes. Put the bolt in and tighten. Release the ratchet strap and you are done.
There is 1 mount on the front of the engine and 2 mounts on the transmission. Put a large block of wood under the oil pan of the engine and jack it up to take the weight off the mounts. Remove the bolt and fasteners on the engine mount. Replace it first. Then replace the 2 mounts on the transmission. The front mount is accessable but the rear mount you have to take the cross member off to get to it.
To replace the motor mount on a 1993 Buick Skylark, start by securing the vehicle on a flat surface and raising it with jack stands. Support the engine with a jack or engine hoist, then remove the bolts securing the old motor mount to the engine and the frame. Replace it with a new motor mount, ensuring it aligns properly, and tighten the bolts to the manufacturer’s specifications. Finally, lower the engine and remove any supports before testing the installation.
Put a jack under the engine for stability. Unbolt one motor mount, and jack the engine up slightly. The mount should be able to be removed, replace it and repeat for the other side.
