I have a 1990 Mercury Sable V6 3.0L, and for me the PCV valve is located on the valve cover behind the intake manifold, to the left (if you're facing the front of the car). With the engine cold (has not run in the past 2-3 hours), locate the PCV valve (it has a black plastic 'elbow' attachment, and is approx. 2" tall). There should be two hoses leading out of the PCV valve.

Since the engine is cold you should be able to use your fingers to feel the hoses and follow them to see where they connect. I know that the thicker one leads to a metal nipple behind the intake manifold (a flashlight and small mirror may be helpful to see the parts behind the intake manifold). I didn't feel any clamps on mine, so when I replace my PCV hose I'll just slide off the old one and insert the new one.

If you have clamps fastening your PCV hose to the PCV valve or intake manifold nipple, release the clamps and then pull off the old hose. I think new hoses are designed to be snug by themselves, so after inserting the new hose you should be done (no clamps required).
