The most common reason for an automobile transmission to shift hard is low Transmission Fluid. Make sure the transmission fluid is at the appropriate level.

When you say jump, do you mean like thump or move or just shift hard or slam......... If it's been driven hard you may want to look at the tranny mounts or engine mounts. Ths will cause movement of the tranny when the torque is supplied and it may be hitting the frame if the mount is deteriorated

My guess would be you shift shaft teeth are worn or your clutch is going out. Also check and see if the shifter is rubbing and catching on anything.
