Plaque build up restricts blood flow. Eventually, a piece of that plaque breaks off and goes to your brain or heart, resulting in heart attack or stroke. You can also get Peripheral Arterial disease which affects your circulation and mobility. Could result in not being able to walk or the loss of a limb.

a stroke occurs. the brain needs nutrients, blood and oxygen so when it can't get these things it starts to have a brian attack, similar to a heart attack.. :)
