Basic stencils are fairly easy to make. All you need is a clear Mylar to use as your base, and a very sharp (exacto) knife.
Your stencils are basically either positive or negative (the hole in the sheet of Mylar or the piece you cut from it)
To start out simple shapes will do you more good... allowing you to use them to make clean edges. As you become more comfortable with using "masks" you can graduate into drawing, cutting and using your own stencils.

This does not always have to be the case, and in the instance of which you are speaking would be for an artist to draw you a custom design and then transferring it onto stencil paper. They could be using a stock piece of tattoo flash that they already have on file, then they simply have to run it through the thermofax machine with the stencil paper to make the stencil. They could draw the design right on your flesh with sterile skin scribe markers or Sharpie's, but these lines wipe off fairly easily, so a lot of artists don't prefer this method. Lastly, they could "free hand" the design right on you, which means that there is no stencil or sketch at all and the artist is creating the design and colors right out of his or her head.

A good solution will keep the stencil on the skin for the duration of the whole tattoo, even if you are constantly wiping (which really you should dab the puddle rather than wipe). Find and buy an antiseptic disenfectant called Dettoll, it is sold in most drug stores and pharmacies, and mix 1 part of Dettol to 7 parts equal distilled water. Voila! Great and easy solution.
Almost any tattoo shop will draw up a design for you and make a stencil. Some artists will draw the tattoo design on with a pen instead. You as a client don't need to provide a stencil.
