Depends on the vehicle and if it's rear wheel or front wheel drive. Some, like the Chevy pickups of the 70's and 80's you could do very easily. Then they began to build vehicles that they didn't want you to fix. Things got put in the way etc. Changing transmission on a modern front wheel drive minivan or SUV can be quite challenging and almost impossible unless you have a vehicle hoist.
The incorrect installation of the torque converter can be a costly mistake.
Pull the transmission and change it. The torque converter is inside the bell housing on the front of the transmission. Only way to get it out is to pull the transmission.
Can be old fluids. You should change it every 50,000. -ALL of it, not just the pan. It could have a broken or stuck accumulator.
it is sugested that you change the fluid every sixty or so thousand, however there isn't relly that much going on in a manul transmission so you don't ever need to change it.
To change from an automatic to manual transmission in a Ford Bronco, you will have to fine a transmission that bolts up to the engine. The transmission mounts have to be changed. The shifter has to be changed along with adding an entire clutch system including a pedal. This will require brackets, drilling in the firewall and possibly welding under the dash.
There seems to be a correlation between the use of my cars by my son and the demise of my transmissions. I am told this is just coincidence..... maybe. I do know though that transmissions wear out over time. They die because of dirt in the fluid (that is why I do not change fluid ever! It increases the change of getting contamination in fluid.) Putting the wrong fluid in destroys them. Heavy trailer loads wear the bands out faster also. Hard use, such as joy riding is hard on the transmission also..... Hmmm, maybe there is a reason the transmission go bad in my cars when my boys drive them.......

Did you use the right transmission fluid or did you forget to maintain your transmission otherwise, your gears are broken and needed to be replaced.. chances are you need a new transmission.
If you clutch is working fine then try to change the gear fluid in the transmission. My Isuzu shifted hard to and the fluid changed helped alot
A bad transmission solenoid will cause hard shifting and can eventually damage the unit. You should change the solenoid if it is bad.

on the top of the transmission it is very hard to get to i just got mine changed at a shop and i watched the guy change it.
Clean pan, change fluid, filter and gasket if no improvement bring to a transmission shop
