Many singers use personal ear monitors so they can hear themselves or a specific mix of instruments and themselves. At many concerts, there is a mix technician that allows each musician to hear what they need to perform better.
The personal ear monitors also help reduce or eliminate the need to have floor monitor speakers which frees up stage space as well as help minimize feedback.

They mic the instruments and singers allow the song to have a good balance of everything in the final version, they add the effects like autotune, the producer of a song would have alot of involvement in what effects are used and to what extend like adding an echo (reverb).

Depends on the positioning of the microphone as well as the cardoid pattern of the mic. Get a goot vocal mic like a shure SM-57 or Sm-58 and you should be OK so long as a loud speaker isn't behind you.
