When installing a bathtub faucet or tub & shower divertor valve, the bathtub spout must terminate above the flood level rim of the bathtub a minimum of 2"inches.This would place the stub out for the spout approximately 6"inches above the rim of the bathtub. Now just install the faucet or divertor high enough so the trim plate will not contact the spout.12 to 15"inches above the spout should be fine.

There is an article on eHow about painting tub surround. http://www.ehow.com/how_4453501_paint-tub-surround.html
Possibly, if you really cleaned the tub aggressively with lacquer thinner or similar de-greaser.
I'm not sure about the sharkbites causing the problem. The root of the problem is that the water flow through the tub filler spout is restricted somewhere, and that is causing the water to gradually rise up to the shower head. The restriction could be anything from solder that seeped into a joint to a kinked piece of tubing or to using too small diameter tubing to the tub filler. Find the restriction, fix it, and the problem should be solved.
In the your moms bath tub.
Because the lid usually has to bend to get it off. If if is too rigid,it will soon break.
The average is 16 inches, naturally there are some lower and some higher, according to requirements and design.
A tub faucet acts as a shower and can be used to bath in the bath tub. It is a pressure valve which makes the flow of water through it constant and gives a nice shower.

The height of a tub faucet should be about 40 inches, give or take a couple of inches, depending upon the height of each member of the family. The spout should be placed 4 inches over the top edge of the tub.
15-16 inches.
Another poster indicated 14 inches as the standard.
It is best top replace the tub facet with the valve that are compatible to avoid leaks or the parts not fitting properly. Home Depot or Lowes should have the parts available.

If the faucet contains the re-direction valve (closes off the water from the faucet and redirects the water to the shower head) you need to rotate the faucet counter clock wise to unscrew it from the plumbing and replace it with a new one. Take the old one to your local box store and they can match it up for you.
A trim kit is just the outside parts that you see. This will make you faucet look better, but it wont affect the function of your old valve. If your shower valve doesn't work right, replace it.
