Exercise increases the heart rate, because the blood has to supply additional oxygen to the muscles.
While any condition affecting the heart is bad, Heart Disease is a general term describing the most serious of these afflictions, such as coronary heart disease, where cardial blood vessels fail to supply the muscles of the heart itself with an adequate supply of oxygen, or Cardiomyopathy, where the muscles of the heart deteriorate.

The heart pumps oxygen-rich blood through arteries which take the blood to muscles. Then when the oxygen is deprived of its oxygen it is pumped back to the heart where it is then sent to the lungs for more oxygen. It then repeats the process. This process is important because your muscles need oxygen to function.

Blood brings oxygen and ATP, which react with one another (oxidation), to the muscle cells. The result of this oxidation process (aerobic) is energy and heat. (Most of the body's heat originates from muscle activity.) Skeletal muscles will not contract unless stimulated by neurons.

Yes, cardiac muscles cells have a very high oxygen demand, since they are constantly in motion. This is why any shortage of oxygen causes angina, or if the oxygen deprivation is long enough, it causes death of the muscle cells - what is commonly called a heart attack.
The coronary artery supply the heart muscles with oxygen enriched blood.
Muscles during exercise need an adequate supply of glucose and oxygen. Heart rate increases during exercise to provide this supply.
