Well, the patient could develop high blood pressure and hypertension possibly resulting in heart attack or cardiac arrest. Or they could just feel very weak and physically drained from insufficient blood flow to vital organs...

It depends on the degree to which it doesn't work. That valve's purpose in life is to allow the ventricle to build up enough pressure to send blood to the lungs. If it's just "somewhat" inefficient, not enough blood will flow into the lungs. You won't get enough oxygen and you'll be fatigued and weak. If it completely fails, no blood will flow into the lungs, your body will receive no O2, and you'll die.

If untreated, this condition can result in enlargement of the right side of the heart and ultimately heart failure and has been linked to strokes. The mechanism by which a PFO may play a role in stroke is called paradoxical embolism.
In the case of PFO, a blood clot from the venous circulatory system is able to pass from the right atria into the left atria via the PFO, and ultimately into systemic circulation.

If the valve between the left atrium and left ventricle can't open all the way, there is the possibility of decreased heart function, as the ventricle would not fill efficiently and cardiac output may be decreased.
