Primers are not water proofers, in fact primers can't be left as the finish coat because they have no durability. If the paint acts as a primer how can you expect the paint to stand up as a true finish coat.
You need to do proper preparation BEFORE you paint the walls. This will include removing all grease and striping off old paint. Then paining the walls with primer and or undercoat before applying the topcoat. The paint tin instructions will detail what you have to do.
If you paint over the "weathered" paint, it will chip and cause the stain to come off. Also, the stain needs a clean sanded surface in order for the wood to absorb it. Otherwise, there is no reason to use stain.
For stains on walls I usually use Mr. Clean Magic Eraser. It isn't harsh or abrasive and really cleans off just about anything (including sharpie marker). DO NOTS: Bleach can change the color of the wall and in some cases strip the paint. Brillo Pads are just too abrasive and can strip paint. 409, Orange Clean, Windex, and Simple Green or just not strong enough to clean off stains or can have a reaction with the wall paint.
First you will want to fill all blemishes on your walls. Sand and repeat if necessary. Also be sure there is no loose paint. Scrape off any loose paint and sand smooth. Make sure your walls are clean. These steps will help ensure the new coat of paint will adhere properly.
