After jacking up the car and removing the wheel , remove the brake pads and caliper. Take the dust cover off of the axle nut, remove the cotter pin and the nut. The rotor can be removed now and the bearing changed. Don't over tighten the axle nut when reinstalling.
Follow all appropriate safety precautions!
For the front (rear similar, except no axle nut)
Remove the wheel, the brake caliper, the rotor, and the axle nut.
Turn the wheel if needed to get access to the three bolts holding on the hub.
Remove the three bolts. Unplug the ABS harness.
The hub most likely will be stuck in place by rust and other corrosion.
Use a punch (not a screwdriver) and attempt to rotate the hub (whack on the triangle shaped part). When it rotates loose, then you can attempt to pry or whack it out.
You may also need to use the punch and whack once or twice on the end of the axle shaft to loosen it. DO NOT DAMAGE THE AXLE THREADS. If you do, you might be buying a new axle shaft.
After the hub comes out, you may need to negotiate the wiring out around the axle shaft.
Installation is the reverse of removal. Use a torque wrench and service manual to properly tighten the axle nut or you WILL be doing the job again in 6 months.
Also do everything in your power not to stretch or stress the new ABS wiring. Hook it up just before you put the wheel back on.
If you have no one to help you hold the wheel while torquing things.. reassemble the whole job except for the axle nut. Set the car on the ground (in park) and then tighten the nut to the specified torque.
Changing a wheel bearing safely requires a large press. On a 1995 Jetta the wheel hub assembly must be removed and the bearing pressed out, just as the replacement unit needs to be pressed in.
No, the front hubs are sealed units on a Grand Cherokee.
In the brake rotors.
They are a sealed bearing and are not greaseable.
Change them. They are not serviceable.

It shouldn't require pressing to remove the bearings.
A Grand Cherokee is either rear wheel drive or four wheel/all wheel drive.
