located on passenger side of vehicle under hood inside large black circilar box with ac lines going into it with wire jack connector visible - the motor may be still be good - it could also be the fan switch or resistor - check motor by touching exposed prong after removing jack with a hot test wire.
I have a 1996 Astro van and I'm having trouble too. My fan motor sticks. It has for a while. Its not the heater but the fan that has draged. I can get out and pound on the casing, and sometimes it starts up.
You'll find the heater blower motor resistor in the heater/air conditioner compartment. This compartment is location underneath the dashboard. This is the part that allows the heater blower motor to change the fan's speed.

in the owners manual, there is a section on fuses and is labeled with numbers (how many amps, location, etc.) Fuse box should be located under the hood. Hope this helps
