A parent that is involved in their child's life and shows an interest for what is going on in school is a way of being nice to a teacher. It shows that you are approachable and if you kid has a problem, the teacher can more easily reach out to you and together you can work it out to the benefit of your child.
If you are a student, you can address the teacher with respect by calling them sir or mam, depending on their gender. Depending on your school, you should also use the proper Mr, Ms, Mrs, Miss, etc. Some schools find it appropriate to be on first name basis between teachers and students.
If you are trying to be extra nice, you can compliment things that your child has learned, you can praise them for being a good educator, or even compliment them as you would any other person. "I like your coat." " That is a very nice pair of shoes, you have good taste." etc. Do not over do, because then you might come of as creepy. The relationship between parents/teachers and students/teachers usually remains a professional one.

Some things to write in your teacher's birthday card could be to wish your teacher a very happy birthday and that you hope he/she has a wonderful day. You could also say that he/she is a great teacher who you admire and respect.
Well honestly I have never met her but when I talk to people who have they say she is very nice. Also, I have read nothing but good things.
