Loosen the lug nuts slightly while the tire is still on the ground, then raise the rear of the vehicle with a jack and secure it with jack stands. Once elevated, fully remove the lug nuts and take off the tire. Remember to replace the lug nuts in a star pattern when reattaching the tire for proper torque distribution.
