I am currently changing a head gasket in a 93 Toyota 22re. I'm willing to share info on the process. I had to grind one of the head bolts off to get it out, I'm also replacing the timing chain.
head gasket is leaking into oil

On the fuel rail, between the throttle body & the head. If you look down between the two you can see them.
when you removed cylinder head or when they are noisy . do it when cold ( 8 thou for intake )and (12 thou for exhaust )
last i heard yes you do so it would be best if you did so.

it should be attached to the back of intake manifold ad to the back of the cylinder head
Torque as follows 53-63 ft/lbs. in 3 steps
Torque the head bolt on a 1993 Toyoto 22RE three times at 58 foot pounds of torque. The 22RE head bolts are not torque-to-yield (stretch), and are re-usable. Torque is 55-63 foot pounds. Thoroughly clean the bolts on a wire wheel and inspect for pitting, especially in the threads and replace as necessary. It is also recommended to re-torque them after about 1,000 miles. ChaCha!
