you have a faulty main sensor..common problem in the integras.you can either replace it or try to fix it by re-soldering the circuit board.easy cheap fix.
Hard to start when warm for your year of Integra is most likely due to the Main Relay malfucntioning. High mileage Acura/honda of the early 1990s are prone to this problem. The part is fairly cheap from your Acura/honda dealer and it takes about 10 minutes to replace.

could be a bad 02 sensor. does it warm up properly when running on the highway?- could be a bad thermostat causing it to run too rich because its trying to warm itself up.
there are two possibilities. in both the outside temp is (very) low and: 1. your thermostat is stuck on open. 2. you have no thermostat. In both situations the car will eventually warm up rather slow and temperature will decrease proportionally with increasing the speed.
