You either have a bad or loose pulley, a stretched belt or a tensioner that isn't working right.
Is the secondary radiator fan coming on when the AC is turned on?
Toyota has released a TSB on this . You have to take it to Toyota and have the catalytic converter replaced and the ECM reflashed. And if you have over 80000 miles on it, you also have to pay for it.
Check your brake fluid level.
Most likely one of the pulleys is out of alignment. Be sure to look at the crank pulley to make sure it is rotating in a true circle.
Hi, I have have the very same problem. I have a 1991 Chev 3/4 tonne. The long stud (bolt) keeps turning and turning ...and the pulley has become loose but won't remove...and the stud keeps turning without coming off even though I put a pry bar behind it at the same time as using my rachet! Rodney

If you meant harmonic balancer keeps going bad you probably have a bent pulley.
