Being it is a b2200 with a carburetor with the mentioned problems here is what to do.Remove the air cleaner housing and clean the carburetor and linkage and especially the choke mechanism.Use carburetor cleaner on the inside of the bowl and choke diaphragm area.I use brake cleaner on the outside of the carb to clean the cable because it will remove heavy build up and dry clean.Then use some liquid wrench and spray on the moving linikage and especially the choke and cable or even the graphite powder.What happens is the choke sticks in low idle or high with these carburetor and can cause the problem.I would recommend removing the egr valve as well and cleaning it being it provides better access with all the above removed and clean with either cleaner above and a brush and make sure you reinstall the gasket exactly as it was removed.It has three 10 mm bolts,one vacumm line,one electrical plug and can effect the trucks idle cold or hot.It is mounted below the carburetor on the right hand side at a angle and about a fist size.Idle would not be necessarily affected by plugs,wires,distributor cap or heat riser unless it has not been serviced in a long time in most cases or the parts are worn.Hope this helps,I have owned my b2200 for 23 years I know this truck pretty good on symptoms.

a lot depends on the engine??possibly a problem with the choke,or a solenoid if a diesel.

it depends on what kind of fule delivery system you have. if it is carborated then it could be because your idle screw is not set high enough and carb engines have to warm up befor they run good in cold weather. if it is carb, engine YOU MIGHT WANT TO ADJUST THE SCREW AND LET RUN FOR ABOUT 5 MIN IN THE COLD. IF IT IS A THROTAL BODY THAN IT COULD BE A HOLE IN ONE OF YOUR LINES AND WHEN IT IS COLD OUT IT WILL CAUSE IT TO ACT UP MORE NOTICABLY THAN IN WARM WEATHER.
