Usually when you are driving down the road and you punch it, your RPM's will jump then go back down. I had this problem in my 1988 S-10 and about 2 weeks later the clutch went.
Slipping, chattering, or grabbing.
Slipping of the clutch and a peculiar smell are the first signs of a clutch that is worn out.
you dont add clutch fluid. If the clutch is slipping that means that you need to buy a new clutch

when you find that when you acelerate you hear the engine noise but cant feel the increase in speed means that clutch plate is worn and is slipping.
if your clutch is slipping it probably means your friction disk is near dead or your clutch master is near dead or both. Which means that it needs to be replaced.
If the revs decrease as the speed decreases (in 3rd & uphill) then its not slipping, you don't need a new clutch, but if you are in any gear and the revs are increasing while the speed is decreasing, then the clutch is slipping.
If you notice slipping when starting off or changing gears after the clutch is completely released, if the pedal rises almost to the top before the clutch engages, if you have difficulty changing gears, or if when you press the clutch the clutch does not disengage completely.

Clutch disc is worn out. Not knowing what vehicle you have, most are hydraulically operated and there is no adjustment. Time for clutch replacement.
