I am going through the same thing on a 2003 taurus. this is really a pain in the rear. As far as i can tell, i need to remove the entire steering column. I am out of daylight today and as soon as i get done I will post the results here.

Remove the ignition retaining ring. Pull the ignition barrel out. Remove the wiring harness from the back of the ignition.
You will need to remove the retaining ring at the top of your 1995 Dodge Grand Caravan ignition switch. Slide the ignition switch out and remove the wiring harness. Reverse the process to install the new ignition switch.
