on my Nissan tarrano the horn's are located behind the indicator units, to access themyou need to remove the indicator units and surrounding plastic grill. There is a horn oneach side of the vehicle, one will (should) be a high note and the other a low note. You'll need a ring spanner to release the nut on the back of the unit. The single cable can be simply pulled of one the unit is released. When replacing ensure you put the washers back in place to stop the unit coming loose.
Newer horns are located by the hood catch mechinism and much simpler to get to. They alsohave two cables (earth no longer supplied by chassis).

Could be a bad horn, bad relay or possible a bad horn ground. Try touching a wire straight from the battery (+) to the horn. If it sounds bad you need a new horn but if OK it's probably a dirty ground or bad relay. Try tightening the horn itself down a little to insure it's getting a good ground. Still sounds bad its probably the relay. Replace it. If none of this helps... a bad steering wheel switch maybe? Hope this helps.
