Oil in the coolant is never good. Unless it was poured in the rad by mistake (flush the rad and replace the coolant), it means there is a serious leak in the engine that needs to be repaired immediately. The usual causes are: head gasket leak, warped/cracked head, intake gasket leak, warped/cracked intake. More rarely, the engine block's water jacket may be cracked or rusted through, in which case the entire engine block must be replaced. Have the problem diagnosed immediately, the longer the problem exists, the more damage is done and the more it will cost to fix.

If the head is warped the head gasket will not seal until you have the head machined. It must be perfectly flat before the head will seal to the gasket.
