An indiviual letter in a punnett square representing a gene is called an allele.

The two genes ( one from each parent ) together is a genotype.

There isn't really two forms of a gene.

The terms gene or allele can be used interchangably in most cases.

The gene locus on each parental chromosome holds the allele/gene passed by that parent to it's offspring. The are the gene pair...or alleleic pair at that gene locus.

A gene is made up of two alleles. For example, a gene could be Aa. The two alleles present in this gene is "A" and "a".
