Pus that usually accompanies an infection is composed of dead bacteria within white blood cells. And thus the swab of a skin wound full of pus is UNlikely to yield a positive (defined by bacterial growth in the culture medium) result. In short live bacteria is needed for a culture to be reported as positive. Not all samples taken for culture contain live bacteria, even in the presence of an infection (it is just the live bacteria are too deep to be accessed by the sampling technique used. Diagnostic problems arrive in psychiatry because the diagnosis of a mental disorder is based on psychological symptoms some of which are called psychopathological phenomena. Psychological symptoms and psycholopathological phenomena (gr. phenomenon - that which appears) are ideas which it is not possible to prove them on a physical basis. One can only ponder in mind if such an idea applies or not. The same is true with symptom-based medical diagnoses (e.g. fibromyalgia, tension- headache, fatigue syndrom ... ). Symptoms are perceived by a person in mind but can`t be proven to exist on an objective level. This makes the great difference between certain medical diagnosis which can be proven on an objective level (physical level) and psychiatric diagnoses (diagnoses of mental disorders) and symptom-based medical diagnoses which it is not possible to prove on an objective level (physical level). With the philosophy of Immanuel Kant out of the "Critique of Pure Reason" the difference of the medical diagnoses and the psychiatric diagnoses can be explained in detail and although the consequences following thereof.
