Nothing at all! Lots of elements have more than 18 total electrons. In fact, every element with an atomic number of 19 or more has more than 18 electrons!

But probably what is really being asked here is in reference to 18 valence electrons. The 18-electron rule, which is essentially the equivalent of the octet rule applied to transition metals, says that transition metals (that have 10 d-orbital electrons) are most stable when they have 18 electrons. Nonetheless, nothing in particular happens if they have more or less except that they are more reactive and such a configuration will probably not last long before reacting or decomposing to form something else.
