It could be as simple as the slit in the plastic mouthpiece has not been cut all the way through. That was my problem with a recently purchased Camelbak Eddy from Target. After already returning one, I was frustrated that my second one had the same issue. After reading around, I realized that the slit had not been cut, I took a knife and carefully pushed on the groove where the slit should be- not too much pressure. It popped right open.
