The conflict of Uglies is when it came time for Tally to turn Pretty, she couldn't because Shay ran away. The choices she had were to stay Ugly or show them where Smoke is.When she got there she realized that when they have the operation they don't just change your look but they create lesions in your brain that make pretties think differently.

Tally has to make a desicion whether she wants to stay ugly for life or turn in her firend and betray her to have the operation to turn her pretty. But then when she gets to the Smoke where Shay escaped to she learns that the operation that truns uglies pretty also alters your brain.
