The only known sword to be "destoyed" is the Giants Sword in the Legend of Zelda: Ocarina of Time. The sword will break and will still be able to do some damage, but not a whole lot. There is an upgradable sword of the Giant Sword only obtainable after completing a series a side quests.

U have to find the master sword in kokori forest and break the "mirror" puzzle.
You go to the dungeon of mystery and beat the boss battle. After that you find a glowing rock. You breahe rock to bk the rock and the sword appears. It might take a while for the rock to break but it does eventually.
An unbalanced sword can give you pains in your arm and will break if hit in the right spot. It might feel awkward in your hand as well. I read this off of the novel Eragon so i dont know how real it might be.
Depending on the type of shield. A smaller buckler would absolutely hurt and even break depending on the size of the sword. A larger shield that is more like a door would not hurt much at all. It generally varies with the size of both sword and shield.
