This is a difficult question to answer because there are so many variables. First, let us define a Coma. A Coma is when the patient does not respond to painful stimuli. If you put a red hot iron on their skin, they would not even flinch. They are completely unresponsive. There are six levels of coma, and this one is the most severe.

A coma can be caused by head trauma, but it can be caused by other things as well, such as a diabetic coma, overdose of drugs, complications of epileptic seizures, alcohol poisoning etc. Sometimes a person is kept in an artificial coma after surgery so that they do not 'fight' the ventilator or breathing machine while they are in the Intensive Care Unit.

Head trauma may cause a transient mild concussion from which the patient recovers quickly. But some head injuries can be so severe that if the patient survives, it is in what is called a Persistent Vegetative State. That means they cannot communicate or help themselves in any way - just like a vegetable.

So comas may be mild or severe, depending on the cause. And a head injury may be mild or severe, depending on how bad the trauma is.

If I wear a motorcycle helmet and I have an accident will my insurance cost be lower than they would if I wasn't wearing a helmet?

Higher because they will see it as you are more of a health liability because you don't wear your helmet, thus more head trauma is likely in the event of a crash, than if you do wear a helmet, where less head trauma is likely. The less head trauma, the less medical expenses they will have to pay, so the less you will have to pay.
