Hetalia is loosely based on history. In general, it is not fully historically accurate.
Some aspects of Hetalia are closely based on real historical events such as the Russo-Japanese War. However, some of them are not depicted as accurately. For example, Spain was not a unified country when it took over Southern Italy. The part that controlled Italy was called the Crown of Aragon. However, Hetalia depicts this as South Italy (Romano) moving over the Spain's house.
Some controversial historical events in WW2 are also not mentioned in Hetalia, such as the Holocaust and the Nanjing Massacre. The damage done by the Allies are also omitted in Hetalia, such as the dropping of the atomic bomb on Hiroshima and Nagasaki. Although some fans argue that these catastrophic events do not belong in a comedy, some people think that it is wrong to remove all the destruction in WW2 as it is a form of bias. Some people even believe that it is wrong to base a comedy on this recent, dark phase of human history.
In general, Hetalia can teach you a thing or two about history in a fun manner and spark your interest in this academic subject, but it is not meant to be a substitute for a History textbook and it is not 100% accurate. Do your research as you're watching Hetalia and know the facts.
