Technically, devices such as modems used for that. At a transmitting exchange point, modem converts digital data into a Logic 1 and 0. A high tone represents logic 1 and a lower tone for logic 0. (This process of modulation is known as Frequency shift Keying), At the receiving end the exchange modem performs the reverse operation (demodulation).

The digital signals are now multiplexed (shared with many others) and sent down fibre optic cables in the form of laser light pulses. The bits are tagged with your digital address and multiplexed with yet more e-mails ,pictures,music, tv programs and any other data needing to be sent. The capacity of these cables is truly mind numbing. At the other end all these signals are demultiplexed in stages and routed to the recipient.
