To left click on a Mac you need to place the Index finger (Forefinger) on the left side of the mouse and press down firmly. Similarly to right click requires the middle finger to be placed on the right side of the mouse and press down firmly. The choice of fingers used can be varied but the index and middle combination are the most common.
The real question is - How do you left click on the Mac Keyboard without a mouse and with the trackpad left click preferences unchecked? I haven't been able to find out an answer outside of getting a Mac mouse.
For windows XP, left click the start button, left click control panel, left click appearance & themes. On the left side of the window it should say mouse pointers, left click that. From there you can change your pointer. On windows Vista, Right click on the desktop, then left click on personalize, then left click on mouse pointers.
Right-click means press the right mouse button. Left-click means press the left mouse button.
The Mighty Mouse and Magic Mouse both support right click.
Move there mouse cursor over the icon and left click.

You need to enable right click. Open System preferences, click on mouse, and make sure the secondary click box is chosen. Choose left or right.
