Heat in short (e.g. 2 minute) intervals, stirring regularly, until the water is steaming. Even if these steps are followed, bubbling won't be obvious. The best way to make sure that water is boiling is use a thermometer. At sea level, water boils at (212° F; 100° C) and that temperature goes down as you go higher in altitude.
