Yes we do because fossil fuels contain carbon and hydrogen, which both burn with oxygen from the atmosphere to produce heat which is used to drive an engine.

The heat comes from the formation of chemical bonds between carbon and oxygen, and between hydrogen and oxygen. The carbon dioxide produced is output into the atmosphere in much the same way as sewage used to to be output to run down the gutter in towns 200 years ago.
