Can clinical notes be added to personal health records or do they need their own file in a workers' compensation and why?

This likely varies from state-to-state. In my home state, the clinical records are a part of the permanent record for any work-related injury. To be honest, though, there is not much interaction, at least here, between the patient's personal physician and the occupational physician. The end result is that there is generally a separate record for the work injury, although that is not intentional. It is important for the occupational doctor and the primary physician to be aware of each others' work.
