It all depends on where you go to. I went to West Virginia University and we offer a program called Multidisciplinary Studies in which you chose 3 minors and they equal up to be your 1 major. I should know, I graduated with that degree last December. My best advice to you would be to dig into your school's undergraduate course catalog or go seek your academic advisor.

It depends on the college or university and their policies. Some institutions may require a declared major after the accumulation of a set amount of completed credits whatever they determine that number is. You would have to check with the school of your choice.
