There are a number of major and minor conflicts in the book titled 'my sister's keeper' by Jodi Picoult.

The main conflicts include:

1) Between Anna the protagonist and her parents:

Anna's older sister Kate had been diagnosed with acute promyelocytic leukemia at a very young age. As there is no cure for it the only way that Kate's life could have been prolonged is if there had been a perfect donor for the various needs of her body depending on the stages of her cancer.

Brian and Sara Fitzgerald, Anna's parents, had conceived Anna through a very unusual method, She had been scientifically designed as a fetus to be the perfect donor for Kate's various needs as Anna develops.

By the time she turned thirteen, Anna had undergone numerous procedures in order to help her sister Kate whenever needed. But just as any adolescent would react, Anna decides its time for her to take charge and total control over her own body and what happens to it. Though the plot reveals that it was Kate, her sister who had convinced her to do so, the reader is made to think that it is Anna's decision independent of her sister's opinion.

Therefore she decides to obtain medical emancipation from her parents so that she would not have to undergo any more procedures against her personal will. She seeks help from Campbell Alexander her attorney whom she forms a bond with as the book progresses.

2) Between Anna's parents:

Brian and Sara Fitzgerald, Brian begins to support Anna's decision to obtain medical emancipation from her parents and this gives a rise to a cold war amongst the entire family, mainly between Anna's parents as her mother, Sara Fitzgerald though she loves her children equally seems to pay more attention to Kate's needs due to her medical condition. So for her to lose her husband to the side which could potentially cause the death of her daughter Kate is very hard to accept and she begins to turn against her husband as well.

The minor conflicts include:

1) The conflict between Anna and her mother Sara Fitzgerald when she is notified that Anna wants medical emancipation.

2) The conflict between Anna's attorney, Campbell Alexander and Julia Romano who is Anna's guardian ad litem for a brief period as they meet after fifteen years past their break up in high school.

3) Sara Fitzgerald's oldest son, Jesse Fitzgerald, he is extremely rebellious, and since his parents always paid attention to Kate's needs more, he feels strongly against them for various reasons.

4) Kate's struggle to survive and the emotional trauma she has to go through before her contradictory views were actually being heard by her paranoid and overprotective mother, Sara.
