I was told that the phrase originated with the famous jazz player Charlie Parker. Whose drug and alcohol addiction had caused massive damage to his internal organs, including severe stomach ulcers. Supposedly he laughed at something so hard it caused his ulcers to rupture and perforate his stomach lining which lead in turn to his heart attack and death. Giving origin to the term Bust a Gut.
