Sounds like you may lose a best friend. But you shouldn't feel guilty just do what feels best for you. If you ask your best friend first then I guess you would have to stay away if she told you to, but if she said it was OK then you could have a lover and still keep your friend. You could test first by going out with him, discreetly, and in a few weeks to months ask your friend if it is ok, if not then lose your best friend. no simple answer.
