Probable reason is your water parameters and other conditions are not right for the fish. These fish (Pangasius hypophthalmus) are usually sold as immature juveniles. They do not usually last long in the aquarium because they usually die of organ failure caused by being kept in the wrong conditions. They are not sharks but large (edible)catfish and under the right conditions will grow to over 36 inches, so they need a minimum sized tank of around 10 feet long with suitable height and depth to match the length. They need a pH of around 7, a dGH of around 10 and a temperature of around 75F. Maybe now you can understand what is wrong with your "Iridescent shark"

The iridescent shark is not actually a shark, it's a shark catfish. This fish lives in the waters of Southeast Asia and reproduces by laying eggs.
The name tells you. They change.
No. Once a fish has lost an eye it will not regrow.
Iridescent shark, also known as pangasius or basa fish, is a popular choice in many cuisines and is considered nutritious. It is low in fat and calories while providing a good source of protein, as well as essential vitamins and minerals such as omega-3 fatty acids. However, the nutritional quality can vary depending on farming practices, and concerns about contaminants in some farmed fish may affect its safety for consumption. Overall, when sourced responsibly, iridescent shark can be a healthy addition to a balanced diet.
