If the card is in poor or 'played' condition, then it does not matter if while it is in a card sleeve, it is indistinguishable from the other cards in the deck.
So of course, you would not be able to use it in an unsleeved deck, if at any time you can identify it just by looking at the back. But assuming it is not creased so badly that it changes the shape of the card sleeve, then it should be fine.
