No, Dimensional Fissure only affects monsters that are sent to the graveyard while it is active - it waits for anything to try to send a monster to the graveyard, and banishes it 'instead', it does nothing to monsters already in the graveyard.

While flipping a monster face-down in response to its activation, will generally not stop the effect's resolution, face-down monsters can't activate their effects at all. (Sole exceptions; Big Shield and Mid Shield Gardna) It is the act of being turned face-down that will send the Equip Spell Card to the graveyard, therefore one way to look at it is that Inzektor Centipede is already face down by the time the Equip hits the graveyard. Because it is face-down at that time, it neither 'sees' the Equip hit the graveyard (so its trigger condition was never met) nor can it activate while face-down anyway.

that is not how it works. you need to have the card they make when they fuse. for example, three blue eyes can be, as you put it, "polymerized" for the blue eyes ultimate dragon but you need to have the ultimate dragon card in your fusion deck already. if you didn't already know your fusion deck is put to the side during the game and you can only summon the monsters in it by fusing the monsters in it's text by some means. one more thing. all monsters that go in your fusion deck say fusion in bold above the monsters listed on it.
